I do this currently with m0n0wall and FreeRadius, you just need to build a
front-end to the radius server which ties back to mysql for you to have ALL
that functionality.

I built one from the ground up... and it works flawlessly for quite a number
of my clients, if you guys are willing I could make up something for you,
however this does require additional hardware for the radius

Of course, the best-known free (open source) Radius server is FreeRADIUS:
http://www.freeradius.org/ - which can be extended by, if I remember
correctly, Python scripts.
