Andrew found it.  You need <media> defined if you want to use <mediaopt> 
in the config.

I think you need to add the media option into your config, testing on my
intel nic (em driver) It looks like you have to specify media and
mediaopt for the setting to be effective

              <media>100BaseTX</media>
              <mediaopt>full-duplex</mediaopt>
              <spoofmac/>
              <ipaddr>206.xxx.xxx.18</ipaddr>
              <subnet>28</subnet>

         https://gateway/exec.php page, (ifconfig em1 media 100BaseTX
         mediaopt full-duplex) it works as expected.  Just not on reboot
         from the config.xml...
