IMHO, the hosts file is solution works but is ugly!

Whatever feature you add to m0n0wall:

1) it consumes more and more memory;
2) it takes a lot of time to be parsed by PHP.

Solution: use a decent Web browser (Firefox for example) and block ads 
at the application level, not at the routing level!

Another option would be to use an external centralized source (database) 
and generate firewall block rules on the fly, without having to store 
them in the config.xml file...
