It looks like a CFDISK.1D from pcengines
(http://www.pcengines.ch/cflash.htm). The pcengines page shows a
standard HD power connector, the adaptor is also available with a
floppy power connector.

The disassembly of a HD may not necessary, the hole placement
(according to pcengines) is the same as the holes on the bottom of a
HD. You would need to use nuts and bolts (or metal MB stand-offs and
nuts)- obviously the holes in the board are not threaded... 

But, this also assumes that the HD mounting bracket in the PC allows
for screws from bottom...
