What is your physical connection to the internet gateway ?
Are you putting the fw behind a cable modem?
If it is a cable modem:
1. turn modem off and m0n0wall off.
2. Turn CM "ON' and wait for ~2minutes
3. Then turn m0n0wall 'ON'
