Yes, everything is opened outgoing. But I'm not using 1:1 NAT like you. I'm doing port forwarding.
(Inbound/Server NAT in M0n0wall) Does that have an effect maybe?

I don't have any problems with DCC on my MailScanner/SpamAssassin server that is behind a m0n0wall
(1-to-1 NAT).

Did you open port 6277 UDP outgoing?
