Next thing I'd try in the process of narrowing things down is a packet
sniffer on the web server, and off the inside of the m0n0wall LAN
interface if possible (if you have a SPAN port capable switch).  If
it's a Windows server, Ethereal, if it's *nix, tcpdump will suffice.
