I'm also using 1.1b15, Vonage and a VT1005V. My setup:

Internet ---cable modem --- m0n0wall --- 8 port switch --- (wan)-vt1005v-(lan) not used

No specail m0n0wall rules or changes other than traffic shaping that is not required but very
desirable.
